Registration Number Application
1. Complete this
application and send the completed form with:
- Five color photographs (head
with eyes visible, left side, right side, front and rear).
- Copy of import documentation,
if directly imported.
- Copy of DNA profiling.
- Copy of Microchip
identification.
- Critique Registration Fee
($30.00).
- Pedigree, extended to five
generations, or as far back as is known.
- Clear copies of all
registration certificates.

2. Once the completed application, photographs, import documentation (if
appropriate), DNA profile, microchip identification, fee, pedigree and copies
of all other registration certificates are received by the Registry, a Critique
Case Number (CCN) will be assigned and this Lhasa Apso may be presented for
evaluation by three qualified judges approved by GLAPP.
3. Once all the rules and requirements for a Critique Registration Number (CRN)
have been met, all necessary fees have been paid, and the Lhasa Apso qualifies per examination by three judges and the
registration review panel, a Critique Registration Number and Registration
Certificate With Pedigree will be issued.
Please
exercise care in completing this application. Incorrect certification below may
result in rejection or cancellation of this Critique Registration Number.
